8 The regular meeting was called to order at 6:00 p.m. by Chair Hansen.

20 V. REPORT BY ACTING DIRECTOR OF PLANNNING AND ZONING:
21 BAHAREH WOLFS: Reported Alton PCD and signage relocation for Downtown at the
22 Gardens was approved at the April City Council Meeting.
23 VI. APPROVAL OF MINUTES
24 Board Member Rosenkranz made a motion to approve the minutes from the February 11, 2014
25 meeting.
26 Board Member Josephs seconded.
21 Motion passed 6-0.
28 Chair Hansen asked for any ex parte.

31 Those preparing to give testimony were sworn in.
32 1. Recommendation to City Council (Public Hearing) -QUASI-JUDICIAL HEARING
33 PPUD-13-11-000033: Church in the Gardens Rezoning
34 A request from Church in the Gardens, Inc. to rezone a 3.23-acre site located at the northeast comer
35 of Holly Drive and 1-95 from Residential Low Density-3 (RL-3) to Planned Unit Development
36 (PUD) with an underlying zoning designation ofRL-3. The Applicant is also requesting a Site Plan
37 Amendment in order to increase the overall building square footage from 19,212 square feet to
38 38,299 square feet and to allow other associated modifications.
39 Chair Hansen recused himself and filled out form 8B and step down from the dais.

5 A request by agent Dwayne L. Dickerson, Esquire, on behalf of the Natures Hideaway
6 Homeowners Association, to amend the Planned Unit Development in order to modify the native
7 upland preserve. The subject site is located west of Prosperity Farms Road, 0.17 miles north of
8 BumsRoad.

18 LDRA-14-03-000052: City initiated LDR Amendment for NBOZ Landscape and Signage
19 Deadlines
20 A City-initiated request to amend the City's Land Development Regulations (LDRs) related to the
21 deadlines for achieving compliance with the landscaping and signage requirements of the Northlake
22 Boulevard Overlay Zone (NBOZ).
23 Chair Hansen opened the public hearing.

32 Board Member Pennell inquired about fmancial responsibility for median surrounding Church
33 of the Gardens property. Discussion ensued. Acting Planning and Zoning Director Bahareh
34 Wolfs provided a status on the gas station on southwest comer of Northlake Boulevard and
35 Military Trail. Discussion Ensued
36 IX. NEW BUSINESS
37 Board Member Savel questioned the attendance policy for board members. Discussion ensued.

This form is for use by any person serving at the county, city, or other local level of government on an appointed or elected board, council,
commission, authority, or committee. It applies equally to membefs of advisory and non-advisoty bodies who are presented with a voting
conflict of interest under Section 112.3143, Aorida Statutes.
Your responsibilities under the law when faced with voting on a measure in which you have a conftict of interest will vary greatly depending
on whether you hold an elective or appointive position. For this reason, please pay close attention to the instructions on this form before
completing the reverse side and filing the form.
INSTRUCTIONS FOR COMPUANCE WITH SECnON 112.3143, FLORIDA STATUTES
A .person holding elective or appointive .cotmly~ municipal, or olher local pulllic o8ice MUST ABSTAIN from voting on a measur~ which
inures to his or her special private gain or loss.. Each elected or appOinted local officer also is prohibited from knowingly voting on a mea-
sure which inures to the special gain or loss of a principal (other than a government agency) by whom he or she is retained (including the
parent organization or subsidiary of a corporate principal by which he or she is retained); to 1he special private gain or loSs of a relative; or
to the special private gain or loss of a business associate. Commissioners of community redevelopment agencies under Sec. 163.356 or
163.357, F.S., and officers of independent special tax districts elected on a one-acre, one-vote basis are not prohibited from voting in that
capacity.
For purposes of this law, a "relative" includes only the officer's father, mother, son, daughter, husband, wife, brother, sister, father-in-law,
mother-in-law, son-in-law, and daughter-in-law. A •business associate .. means any person or entity engaged in or canying on a business
enterprise with the officer as a partner, joint venturer. coowner of property, or corporate shareholder (where the shares of the corporation
are not listed on any national or regional stock exchange).
