7 The regular meeting was called to order at 6:00 p.m. by Chair Michael Panczak.

12 (Second Alternate sitting for Douglas Pennell).
13 Members Absent: Charles Hathaway, Douglas Pennell.

20 Director of Planning and Zoning Natalie Crowley reported House Bill 7207 Community
21 Planning Act will be presented to the board at the end of summer.
22 VL APPROVAL OF MINUTES
23 Howard Rosenkranz made a motion to approve the June 21, 2011 minutes.
24 Roma Josephs seconded.
25 Motion passed 6-0.
26 Those preparing to give testimony were sworn in.

28 1. Recommendation to City Council (Public Hear®gl Quasi - Judicial Hearing
29 PUDA 11 -04- 000028 — Village Sauare II Planned Unit Devdonment H (PUD) Amendment —
3o A request by the property owner, Monet Road LLC, for approval of a Planned Unit Development
31 (PUD) amendment to allow for an increase of medical office square footage on the site and minor
32 site plan modifications to accommodate the additional medical office use. The Village Square H
33 PUD is located on 2.57 acres south of RCA Boulevard and west of Prosperity Farms Road.
34 Randolph Hansen recused himself left the dais and filled out Form 8B.

6 Discussion ensued.
7 Howard Rosenkranz made a motion for approval.
8 Joy Hecht seconded.
9 Motion passed 5 -0.
10 Randolph Hansen returned to the dais.
11 It was announced that petitions 2, 3 and 4 would be presented at one time with individual motions
12 for each item.
13 2. Recommendation to City Council (Pnbiic Hearing) -QUASI - JUDICIAL HEARING
14 SPLA -11 -044)00022 - Old Palm Planned Community Development MM Amendment for
15 Parcel F1 (Clubhouse Parcel) — A request by the property owners, Old Palm Holdings LP and Old
16 Palm Golf Club, Inc., for approval of a major site plan amendment for Parcel F 1 to delete fourteen
17 (14) un -built dwelling units and to add four (4) tennis courts to Parcel F 1. The Old Palm Golf Club
18 PCD is generally bounded by PGA Boulevard to the south, Westwood Gardens to the north, Central
19 Boulevard and Interstate 95 to the east, and Florida's Turnpike to the west.
20 3. Recommendation to City Council (Public Hearing) - OUASI JUDICIAL HEARING
21 SPLA -11 -044)00023 - Old Palm Planned Community Development (PCD) Amendment for
22 Parcel A (Sabal Palm) — A request by the property owner, Old Palm Holdings LP, for approval of
23 a major site plan amendment for Parcel A in order to add ten (10) of the fourteen (14) dwelling units
24 deleted from Parcel F1 to Parcel A, to modify the landscape plan, and to amend the design
25 guidelines. The Old Palm Golf Club PCD is generally bounded by PGA Boulevard to the south,
26 Westwood Gardens to the north, Central Boulevard and Interstate 95 to the east, and Florida's
27 Turnpike to the west.
28 4. Recommendation to City Council (Public Hearing) - OUASI- JUDICIAL HEARING
29 SPI.A -11 -04400024 - Old Palm Planned Community Development (PCD) Amendment for
30 Parcel C (Maiesty Palm) — A request by the property owner, Old Palm Holdings LP, for approval
31 of a major site plan amendment for Parcel C in order to add four (4) of the fourteen (14) dwelling
32 units deleted from Parcel F1 to Parcel C, to reconfigure certain lots, and to amend the design

This form is for use by any person serving at the county, city, or other local level of government on an appointed or elected board, council,
commission, authority, or committee. It applies equally to members of advisory and non - advisory bodies who are presented with a voting
conflict of interest under Section 112.3143, Florida Statutes.
Your responsibilities under the law when faced with voting on a measure in which you have a conflict of interest will vary greatly depending
on whether you hold an elective or appointive position_ For this reason, please pay close attention to the instructions on this form before
completing the reverse side and filing the form_
INSTRUCTIONS FOR COMPLIANCE WITH SECTION 112.3143, FLORIDA STATUTES
A person holding elective or appointive county, municipal, or other local public office MUST ABSTAIN from voting on a measure which
inures to his or her special private gain or loss. Each elected or appointed local officer also is prohibited from knowingly voting on a mea-
sure which inures to the special gain or loss of a principal (other than a government agency) by whom he or she is retained (including the
parent organization or subsidiary of a corporate principal by which he or she is retained); to the special private gain or loss of a relative; or
to the special private gain or loss of a business associate_ Commissioners of community redevelopment agencies under Sec. 163.356 or
163.357, F.S., and officers of independent special tax dstricts elected on a one -acre, one -vote basis are not prohibited from voting in that
capacity.
For purposes of this law, a "relative" includes only the officer's father, mother, son, daughter, husband, wife, brother, sister, father -in -law,
mother -in -law, son -in -law, and daughter - in-law. A 'business associate" means any person or entity engaged in or carrying on a business
enterprise with the officer as a partner, joint venturer, coowner of property, or corporate shareholder (where the shares of the corporation
are not listed on any national or regional stock exchange).
ELECTED OFFICERS:
In addition to abstaining from voting in the situations described above, you must disclose the conflict
PRIOR TO THE VOTE BEING TAKEN by publicly stating to the assembly the nature of your interest in the measure on which you
are abstaining from voting; and
WITHIN 15 DAYS AFTER THE VOTE OCCURS by completing and filing this form with the person responsible for recording the min-
utes of the meeting, who should incorporate the form in the minutes_
APPOINTED OFFICERS:
Although you must abstain from voting in the situations described above, you otherwise may participate in these matters. However, you
must disclose the nature of the conflict before making any attempt to influence the decision, whether orally or in writing and whether made
by you or at your direction.
