The meeting started at 1:11 PM
Nina Nikolva opened the meeting and handed out the comment packets.
Judy Dye submitted a list of comments and asked that Ann go through and respond to
them in writing. Ann Booth asked for clarification on comment # 6 regarding the size the
sidewalks need to be. Judy went over the site plan and explained how the sidewalk
should be constructed. Ann also had a concern about comment # 7 with regard to parking
spaces and wheel stops. There is a plan to ask for a waiver regarding the parking spaces.
Ann asked if the construction plans can be submitted when applying for the permits and
Judy stated they should have engineering plans as part of this process. Ann asked if this
can be made a condition of approval because they are not available at this time. Judy
agreed that the conditions will be written into the site plan approval for construction plan
review and approval since the applicant had not contracted the engineering services as of
this meeting.
Ross Gilmore talked about his list of comments. Ann had a concern with comment #2 on
Ross's list with regarding to putting up a wall to be used as a buffer. Ross stated that he
would like a wall that would be a buffer because the noise could be a problem for the
residents. Ann stated she would need to discuss this with the hospital but she doesn't
really understand how the wall will buffer the noise because it would not be that tall.
Mike Morrow does not think putting up a wall will mitigate the sound. He said the sound
is going to bounce off of the wall and make it worse for some of the residents. Ann would
like to look into increasing the height of the trees. Ross stated that they would possibly
disregard the wall if they planted more trees and shrubs and made the area denser. There
was discussion about where the flight path and where the helicopter will be landing.
Ross went on to discuss the landscaping and asked that the hospital replace trees that
were damaged in the hurricane.
Ann asked for clarification regarding the location of the Palm Tran bus stop.. Mike
Morrow showed her on the site plan and they discussed the specifics of the bus stop
shelter and location.

We have reviewed the following plans and information for the above referenced
project received April 4, 2006:
• Applicant's Response Narrative to our memorandum dated November 23, 2005
prepared by Urban Design Studio.
• Development Application prepared by Urban Design Studio.
• Aerial and Location Map prepared by Urban Design Studio.
• Surrounding Property Owners Listing prepared by Urban Design Studio.
• Conditional Use Overlay Analysis prepared by Urban Design Studio.
• Site Plan and Landscape Plan prepared by Urban Design Studio.
• Copy of Letter from Federal Aviation Administration dated March 30, 2006.
We have the following comments:
• The purpose of this application is to allow the relocation of the helipad from the
current location on the approved PUD -99 -07 Master Plan, to a location as
shown in the submitted CUMJ- 06 -03- 000002 Site Plan. With the relocation of
the helipad there is a net loss of 26 parking spaces and the applicant is
addressed this loss by reducing the square footage of Phase 213 by 5,200 square
feet.
1. Satisfied. The current submittal identifies 43 less parking spaces than required.
The applicant states in their narrative response dated January 16, 2006 that a
waiver was granted for the previously approved site plan allowing 43 less
parking spaces. We have reviewed Ordinance 35, 2000 and concur with the
applicant's assessment.
2. Satisfied. The applicant has clarified the plan view and legend to identify
existing construction, proposed Phase 2.13.1 construction and future phases.
3. Satisfied. The applicant has provided an eight -foot sidewalk from the Intensive
Care Unit (ICU) to the proposed helipad.

4. Satisfied. The applicant states in their narrative response dated January 16,
2006 that the helipad is for landing and take -off only and there will be no
repair, refueling, short-term, or long -term parking allowed.
5. Satisfied. The applicant has provided a copy of the Federal Aviation
Administration letter dated March 30, 2006 and Conditional Use Overlay
Analysis that satisfactorily addresses LDR Section 78- 159(60).
6. The applicant shall revise the proposed handicap ramp located northwest of the
helipad to accommodate the proposed eight -foot sidewalk and five -foot
sidewalk intersecting the proposed type D curb. The applicant shall label the
curb ramp number (CR #) conforming to 2004 FDOT Index 304 for each of the
two proposed handicap ramps within Phase 2.13.1.
7. The applicant shall provide wheel stops for the 26 proposed parking spaces
within Phase 2.B.1 located east of the ICU and for the seven proposed parking
spaces within Phase 3 located north of the rain shelter.
Non - Certification Issues:
1. Not Satisfied. The applicant shall label all curb ramp numbers (CR #)
conforming to 2004 FDOT Index 304 or provide a detail conforming to other
state or federal guidelines.
2. Not Satisfied. The applicant shall indicate on the plans the structural numbers
for the new pavement sections in accordance with Section 78 -499, Table 41 of
the LDR. The applicant shall provide a table indicating the layer, materials,
LBR/FBV, material thickness, FDOT layer coefficient; the SN for the
pavement section, base section, and subgrade section; and the total SN for the
total pavement section and the required SN in accordance with the FDOT
Flexible Design Manual.

Conditions of Approval
1. "Applicant shall copy to the City all permit applications, permits, certifications
and approvals. " (City Engineer)
2. "Applicant shall provide all necessary construction zone signage and fencing
as required by the City Engineer. " (City Engineer)
3. "Prior to construction plan approval and the issuance of the first land alteration
permit, applicant shall provide cost estimates in accordance with LDR Section 78-
309 and 78 -461 and for on -site project improvements, not including public
infrastructure, or landscaping and irrigation costs for review and approval by the
City. The cost estimates shall be signed and sealed by an engineer and landscape
architect registered in the state of Florida and shall be posted with the City, prior
to the issuance of the first land alteration permit. (City Engineer)
4. "The construction, operation and /or maintenance of any elements of the subject
project shall not have any negative impacts on the existing drainage of
surrounding areas. If, at any time during the project development, it is
determined by the City that any of the surrounding areas are experiencing
negative drainage impacts caused by the project, it shall be the applicant's
responsibility to cure said impacts in a period of time and a manner acceptable
to the City prior to additional construction activities. " (City Engineer)
5. "Prior to issuance of the first land alteration permit, applicant shall submit
signed /sealed /dated construction plans (paving /grading /drainage and
water /sewer) and all pertinent calculations for review and comment". (City
Engineer)
6. "Prior to construction plan approval and the issuance of the first land
alteration permit, applicant shall schedule a pre permit meeting with City
staff. " (City Engineer)
7. "The applicant shall provide a letter of authorization from the utility easement
owners authorizing encroachment of the landscape buffers within their
respective utility easements. " (City Engineer)
The applicant is requested to return a copy of our comments with the
applicant's acknowledgement of each comment and the response. Compliance
will expedite the subsequent review.

I recommend approval of the above - referenced application with the following conditions
of approval:
1. In accordance with Section 78 -315 of the LDR, a landscape island is required for
every nine parking spaces located in a row. The applicant shall revise the site plan
and landscape plan accordingly or ask for a waiver.
2. The applicant has requested a waiver to eliminate several shade trees from
landscape islands adjacent to the helipad in order to not interfere with the flight
path. In exchange for this waiver and the waiver to reduce the number of
landscape islands, I recommend the installation of a wall extending along the
entire length of the south property line and north along the east property line tying
in to the existing wall. This wall will help buffer the surrounding residential units
from the noise associated with the operations of the helipad. The wall shall be
installed simultaneously with the construction of the helipad or within six months
from the effective date of approval, whichever is shorter.
3. I recommend the applicant redesigns the proposed parking area, located adjacent
to the ICU building, to create a divider median between the rows of head -to -head
parking spaces. This can be accomplished by reducing the length of the parking
stalls to 16 feet which allows the overhang area to be grassed or mulched. Trees
can than be planted in this strip. Trees would than be located as if diamonds were
used. This will provide additional room for tree root development, help to direct
traffic flow, and match the design of the existing parking area.
4. Please revise the site plan and landscape plan to show the proposed flight path and
the heights of all light poles located within the flight path.
5. Please revise the site plan and landscape plan to show the location of the helipad
identification beacon, the wind indicator, and any associated lighting.
6. I anticipate a Condition of Approval which would require the applicant to replace
all missing or damaged landscaping, as shown on the approved landscape plan for
the property, within six months of the effective date of approval.

Staff has reviewed the above - referenced petition and has the following comments:
Certification Issues
1. The applicant shall provide the petition number on all revisions.
2. The applicant shall list the requested waivers on the site plan.
3. The applicant shall indicate on the site plan the dimensions of the impacted
parking spaces.
4. The applicant shall revise the Site Data table and Phasing Schedule to reflect the
reduction of the square footage for Phase 2B.
5. The applicant shall correct the number of parking spaces that have been affected
due to the helipad relocation, and shall amend accordingly the square footage that
will be reduced due to the loss of parking spaces.
6. The applicant shall revise the "file name" and the "Master Plan Phases 4 and 5"
on Sheet A1.0 to reflect the actual phase that is being modified.
7. The applicant shall indicate the flight path on the site plan.
8. The applicant shall provide a detail of the helipad.
Non - Certification Issues
9. In order to minimize the impacts on the adjacent residential development and
provide additional screening, the applicant shall install a six -foot wall along the
southern property line of the PUD.
10. The applicant shall provide the City with an easement for a Palm Tran bus shelter
along Burns Road or Gardens East Drive.
