Fire Department No comments at this time. Building and Community Services Department s No comments at thi s time. Please be advised that permits will be required for all proposed new work. Police Department No comments at this time. Seacoast Utility Authority 1. The applicant is advised to utilize root barriers to protect the private sewer lateral from tre e roots. Planning and Zoning Department 2. Please change the “zoning” referenced in the data table on Sheet 2 of 9 to reflect an existing zoning of PUD/CG -1 and remove the references to “proposed zoning” and “proposed land use.” 3. Please clarify the ground m aterial proposed for the southwest parking area during Phase I. T his area is shown as dotted parking spaces . Some type of landscaping ground cover should be installed in this area during Phase I to deter parking. Per Section 78 -345(d), Increases in park ing spaces , please clarify what parking spaces are part of Phase I. The number of parking spaces proposed for Phase I appears to exceed 38 percent of the number of spaces required. Please r evise the site plan accordingly. 5. It appears that new utility lin es have been added to the site plan. A private sanitary line has been added along the eastern property line and a new water line is shown along the west side of the property . Please clarify whether easement s will be established for the new line s , as th ey will need to be reflected on the plat application, which is currently pending. 6. Please be advised that t he Build -out date for this project is December 31, 2012 . Please note a time extension request must be submitted 30 days prior to the expiration date, per Section 78 - 61(g)(2). 7. Please provide an electronic copy of your resubmtital. GIS 8. Please submit the CAD open space review fee of $3 00.00. Comments will be provided separately at the time of file review. Forestry 9. Please revise the Phase II landscape plan to change the placement of the Japanese Blueberry on the south side of the southern entrance from Sunset Drive, to avoid conflicts with the stop sign. These plants have been moved from the location shown in the previously approved landscape plan. A lso consider revising the locations of buffer trees in this area to allow Phase I trees to remain in place for Phase II. 10. Please revise the Phase I landscape plan and the existing landscape plan to show the existing Royal Palm at the northeast corner of t he building remaining in place until parking lot improvements in this area for Phase II are needed. There are three (3) Royal Palms currently existing in this location, and only two (2) are indicated on the Phase I plan. 11. Please revise the Phase I and Phas e II landscape plans to address the addition of the sanitary sewer line along the eastern façade of the building, as SUA standards may not allow the placement of the line in close proximity to the existing Live Oak tree and proposed trees. 12. Please revise t he open space, impervious, pervious and sod area CAD files for Phase I to match the proposed Phase I site plan. Specifically, include the landscape area at the northeast corner of the building, as the area is not proposed to be removed to create parking un til Phase II. Prior to issuance of the first building permit for ver tical construction , the Applicant shall execute a unity of title which shall unitize the two lots having street addresses of 4114 Northlake Boulevard and 8955 Sunset Drive. Such unity of title shall be submitted to the City Attorney for approval prior to being recorded. Planning and Zoning Department 2. The permitted uses shall consist of business and professional office. Medical office uses shall not be permitted on site unless a traffic equivalency analysis and a parking analysis are approved by the Dire ctor of Planning and Zoning. (Planning & Zoning) 3. Prior to the issuance of the first building permit for vertical construction , the Applicant shall install a six - (6) foot tall construction fence with a privacy tarp around the perimeter of the constructio n area. (Planning & Zoning) 4. Prior to the issuance of the building permit for vertical construction Certificate of Completion for the parking lot improvements associated with Phase I , the proposed wall along the south property line shall be installed in a ccordance with the approved development plans. (Planning & Zoning ) 5. Prior to the issuance of the first building permit for vertical construction, the A pplicant shall schedule a pre -permit meeting with the Planning and Zoning Department (Planning & Zoning) 6. Prior to the issuance of the Certificate of Completion for the parking lot improvements associated with Phase I , all ground -mounted mechanical equipment shall be screened from view per Section 78 -377. (Planning & Zoning) 7. Prior to the issuance of the fi rst Certificate of Occupancy, all ground -mounted and rooftop mechanical equipment shall be screened from view per Section 78 -377. (Planning & Zoning) 8. All on -site lighting shall be cast downward and shielded from adjacent properties. (Planning & Zoning) 9. Art in P ublic P laces (AIPP) is required per Section 78 -261(c) if the vertical construction costs of all new improvements on the project site are greater than $1,000,000, in which case art valued in an amount of one percent (1 %) of the total vertical cos ts is required. (Planning & Zoning) 10. At no time shall staging of construction vehicles and/or service vehicles occur within a public right -of -way. All gutters, downspouts, vents, and louvers attached to the building shall be paint ed to match the surface to which it is attached. (Planning and Zoning) 12. The Build -o ut date for this project is December 31, 2012, or as otherwise extended as permitted by Code. (Planning and Zoning) 13. Prior to the issuance of a C ertificate of C ompletion f or each phase of construction, the applicant shall obtain a landscape permit for that phase and have the completion of the landscaping certified by the landscape architect and inspected and accepted by the City Forester or designee. (Planning and Zoning) 14. Prior to the issuance of the first Certificate of Completion for the proposed improvements, an inspection is required by the Development Compliance Division. (Development Compliance Division) 15. Prior to the issuance of the Certificate of Occupancy for the new building addition, digital files of the approved civil design and architectural drawings, including floor plans, shall be submitted to the GIS Manager. (GIS, Development Compliance) 16. The off -site landscaping and irrigation improvements within the Suns et Drive road shoulder and the Northlake Boulevard road shoulder and median shall be completed prior to the Certificate of Occupancy for the new addition or within six (6) months from the approval of this petition, whichever comes first Completion for the parking lot improvements associated with Phase I . The term “completed” shall mean accepted by the entity issuing the road right - of -way permit (i.e., FDOT or Palm Beach County). A one -time extension for completion of this condition may be granted by the D irector of Planning and Zoning upon request by the property owner. (Forestry, Development Compliance) 17. The A pplicant, successors , or assigns shall be responsible for the maintenance of the new off - site landscaping and irrigation improvements within the Su nset Drive road shoulder and the Northlake Boulevard median once same is completed. The maintenance of the existing landscaping and irrigation within the Northlake Boulevard road shoulder shall commence immediately. (Forestry, Development Compliance) Po lice Department 18. Prior to the issuance of the infrastructure permit or the issuance of the building permit for the vertical construction, whichever occurs first, the Applicant shall submit a site security plan detailing the measures the A pplicant/contracto r will take to minimize theft during the construction phase. (Police) Engineering Department 19. Prior to the issuance of the building permit for the vertical construction, an infrastructure permit will be required for this project. Prior to the issuance of the infrastructure permit, the Applicant shall provide construction plans, including, but not limited to, paving, grading, and drainage plans along with surface water management calculations for City review and approval. The paving, gradi ng, and drainage plans and calculations shall be signed and sealed by a registered engineer licensed in the State of Florida. (Engineering) 21. Prior to the issuance of the infrastructure permit or the issuance of the building permit for the vertical constru ction, whichever occurs first, Technical Compliance Approval (TCA) for the plat of this project is required from the Planning & Zoning Department, in accordance with the City’s LDR. (Engineering) 22. Prio r to the issuance of the Infrastructure Permit, the Applicant shall provide the approval permit from Palm Beach County for the off -site improvements , including, but not limited to, the proposed curbing and sidewalk. (Engineering) 23. Prior to the issuance of the infrastructure permit or the issuance of the building permit for the vertical construction, whichever occurs first, the Applicant shall provide to the City letters of authorization from the applicable utility companies allowing landscaping and light p oles to be placed within the utility easements. (Engineering) 24. Prior to the issuance of the clearing permit, the infrastructure permit, or the building permit for the vertical construction, whichever occurs first, the Applicant shall provide itemized cost estimates and surety for the project, in accordance with LDR Sections 78 -309 and 78 - 461. The itemized cost estimates shall include all public elements for the project for the on -site and off -site infrastructure, landscaping , and irrigation costs. The co st estimates shall be dated, signed , and sealed by an engineer and landscape architect registered in the State of Florida. Surety will be based on 110 percent of the total combined City -approved cost estimates and shall be posted with the City. (Engineer ing) 25. Prior to the issuance of the infrastructure permit or the issuance of the building permit for the vertical construction, whichever occurs first, the Applicant shall provide a signed and sealed photometric plan and submit a site lighting permit appli cation. (Engineering) 26. The Applicant shall provide a signed and sealed pavement marking and signage plan, or provide the same on the engineering plans. Said plans must be reviewed and approved by the Engineering Department prior to the issuance of the i nfrastructure permit or the issuance of the building permit for the vertical construction, whichever occurs first. (Engineering) 27. Prior to the commencement of construction, the Applicant shall schedule a pre -construction meeting with City staff. Inspect ions related to the infrastructure permit will not be performed until the pre -construction meeting has occurred. In addition, failure to comply with this condition could result in a Stop Work Order of all work/construction activity for the subject develop ment site. Prior to the commencement of construction, the Applicant shall provide all necessary construction zone signage and fencing as required by the Engineering Department . (Engineering) 29. Prior to the issuance of the Certificate of Com pletion for the infrastructure permit by the Engineering Department or the issuance of the first Certificate of Occupancy, whichever occurs first, the Applicant shall provide copies of the required FDOT testings for the City’s review and approval. (Engine ering) 30. The Applicant shall copy to the City all permit applications, permits, certifications, and approvals. (Engineering) 31. Prior to the issuance of the Certificate of Completion for the infrastructure permit by the Engineering Department or the issuance of the first Certificate of Occupancy, whichever occurs first, the Applicant shall provide the Engineering Department with copies of all permits, permit applications , and Requests for Additional Information (RAIs) to and from regulatory agencies regarding issues on all permit applications, certifications , and approvals. (Engineering) 32. The construction, operation, and/or maintenance of any elements of the subject project shall not have any negative impacts on surrounding areas , including, but not limited t o, drainage, erosion, sedimentation, dust, etc. If at any time during the project development it is determined by the City that any of the surrounding areas are experiencing negative impacts caused by the project, it shall be the Applicant’s responsibilit y to resolve said impacts in a period of time and a manner acceptable to the City prior to additional construction activities. The City may cease issuing building permits and/or Certificates of Occupancy until all off -site concerns are resolved. (Enginee ring) 33. The Applicant shall comply with all Federal Environmental Protection Agency and State of Florida Department of Environmental Protection permit requirements for construction activities. (Engineering) Additional conditions are forthcoming pending re submittal. The A pplicant shall provide a written response to all comments, indicating how each comment has been addressed and/or acknowledgement of each comment . Compliance will expedite the subsequent review. It is suggested that the applicant clearly id entify all changes to the plans by either “clouding”, or highlighting, the location of all changes to further expedite the review. City staff reserves the right to make additional comments throughout the review process.
